Community Access Network (CAN)
What is this service?
CAN (Community Access Network) is a community-based, person-centred service that supports adults to live connected, meaningful, and independent lives. The service focuses on early intervention and prevention, helping to reduce social isolation and support people to access community and universal services in their local area.
Who it’s for
- Adults with disabilities and/or neurodivergence
- People who experience social isolation or barriers to inclusion
- People who want support to maintain or improve independence and wellbeing
- Support is strengths-based, flexible, and shaped around individual goals.

- Adult Social Care budgets
- Direct Payments

- Support to access community-based and universal services
- Help building confidence, skills, and resilience
- Small group or one-to-one support
- Support to develop routine, structure, and social connections
- Outcome-focused support that promotes independence and wellbeing

- Are interested in volunteering as a meaningful activity
- Would benefit from supported pathways into volunteering
- Volunteering is optional, person-led, and strengths-based.

- All CAN support
- Support to identify suitable volunteering opportunities
- Ongoing support to sustain volunteering placements
- Support to build skills, confidence, and work-related behaviours
- Opportunities to progress towards employment, training, or further community involvement, where appropriate
How to access CAN or CAN Plus
CAN and CAN Plus can be accessed through Adult Social Care as part of an agreed Care and Support Plan, or by using Direct Payments.
People accessing support, families, carers, and social workers can discuss whether CAN or CAN Plus is suitable based on individual needs, goals, and outcomes.
